Congratulations on your graduation or completion of a major milestone! We want to celebrate with you.

Please fill out this form and we will honor your accomplishments in worship on Promotion Sunday, June 4th.

We will show the names and photos (if you wish) of all those who fill out this form during the service.

The deadline to submit this form is Sunday, May 14th, in order to provide time to prepare a video for June 5th.
